Endoscopy has dramatically changed the practice of gastroenterology and indeed medicine in general over the last several decades. Among the many noteworthy advancements in endoscopy are those in endoscopic treatment of surgical complications, which may include anastomotic strictures, leaks, and fistulas, among others.

What was once an indication with limited number of therapeutic options with regard to accessories and techniques, or even a contraindication to endoscopy, has now become a substantial and integral part of advanced endoscopy and often a surgeon’s saver, as endoscopic interventions are safe and effective as first-line therapies, improving patient outcomes while avoiding the need for further surgeries. In addition, the therapeutic options for managing surgical complications continue to evolve, as novel therapies are developed, tested, and approved, thus continuing to expand the potential and armamentarium of minimally-invasive solutions to a wide variety of clinical disorders. In order to maximize endoscopic results, the selection of the best therapeutic options and development of algorithms for the management of each clinical situation is crucial.

The aim of this Special Issue is to help define a consensus on the most appropriate therapeutic approaches (in terms of efficacy, safety, and cost-effectiveness) to postsurgical leaks, fistulas, or weight regain, to evaluate combined therapies in refractory benign anastomotic strictures in order to increase dilation-free period, and to evaluate outcomes of different endoscopic approaches to postsurgical collections. We welcome both original research and review articles that help to address this exciting and growing area in advanced endoscopy, as well as articles which examine the current state and horizon of endoscopic treatment of surgical complications.

Potential topics include but are not limited to the following:

  • Management of post-bariatric complications, such as strictures, leaks, fistulas, or weight regain
  • Endoscopic treatment of esophagectomy leaks
  • Endoscopic treatment of gastrectomy leaks
  • Endoscopic treatment of refractory benign anastomotic strictures
  • Endoscopic management of entero-enteric or entero-cutaneous fistulas
  • Endoscopic solutions for colorectal anastomotic leaks
  • Endoscopic ultrasound (EUS)-guided drainage of postoperative collections
  • Head-to-head comparisons between endoscopic modalities in the management of leaks and fistulas, such as EID vs EVT or EVT vs stents
  • Head-to-head comparisons between endoscopic modalities in the management of refractory anastomotic strictures, such as dilation vs incisional therapy or dilation vs stents
  • Cost-effectiveness studies
